To complete enrollment into your chosen university program, please email full color high resolution electronic copies of the following documents to apply@chinaunipath.com:

One recent passport-size photo
Your passport (showing your full name, date of birth, passport number, date of expiry, etc.)
Your most current education diploma or academic transcript

Note: Your application is incomplete until you have submitted these documents to us.

Step 3 - Payment

For the final step of your application, please make a payment for the service option you selected in Step 1. Please follow the payment instructions you will receive in your email shortly after you submit this form.

It is very important that the contact details you provided in the above form is correct. Please take the time to review your application form before you make your final submission.
